Champions Leauge matches are to be reported via Assignr using the dropdown tab to select Champions Conference. Game reports must be submitted within 24 hours of the completion of the game..

| Send offs: |
DO NOT KEEP PLAYER OR COACHES PASSES!
Passes of players or team officials that are sent off must be sent on the
next business day to Champions Conference, c/o Pam Whitcomb, 4890 West Ridge Road,
Spencerport, NY 14559
If a player, substitute or team official is sent-off, in addition to filing the
online report below, also fill out a USSF Game report and supplement, and within
24 hours. email a copy to the League Assignor
and the League Coordinator.
If the game did not start or did not reach it's full time please also email a
USSF report/supplemental to the League Assignor
so that those issues can be tracked.
If your report indicates that there was a possible referee assault, abuse or harrassment;
please email a copy of the report to the SRA (Tom Langelotti)
prior to sending a report to the league.
